Hydrogen breath test for the diagnosis of lactose intolerance. Epidemiology, predictors and treatment.


Resumen

Introducción y objetivos: La intolerancia a la lactosa (IL) induce una clínica inespecífica superponible a trastornos funcionales. Es útil conocer características clínico-epidemiológicas y síntomas predictores así como determinar la asociación con otras patologías y analizar el uso del tratamiento sustitutivo.

Material y Métodos: Estudio retrospectivo de pacientes con TAHL patológico (TAHL+) desde 2010-2014. Se recogieron datos clínico-epidemiológicos y prescripción de sustitutivo.

Resultados: De 2038 pacientes, el 30% tuvieron TAHL+. Edad media 35 años. En el análisis multivariante los hombres presentaron un 33% de más riesgo para TAHL+ (OD 1.33 IC 95%: 1.03-1.71). Existe un incremento en el número de test realizados hasta 2012 y una disminución progresiva en la incidencia de TAHL+. El 42.8% de los TAHL+ fueron encasillados inicialmente como síndrome de intestino irritable (SII). El 33.8% de pacientes con enfermedad inflamatoria intestinal (EII) presentaron TAHL+. Los síntomas principales por los que se indicó el test son: Dolor abdominal y diarrea. Durante la realización del test, el síntoma presentado con más TAHL+ es meteorismo (35% p=0.03). La prescripción de sustitutivo se realizó en el 36.5% de los pacientes con IL.

Conclusiones: La IL tiene una frecuencia moderada en nuestro medio del 30%, con una edad media 35 años, y es más frecuente en hombres. Existe una disminución progresiva en su frecuencia a pesar del aumento de test solicitados. El 42.8% con TAHL+ fue inicialmente diagnosticado de SII y un tercio de los pacientes con EII presentaron TAHL+. El meteorismo es predictor sintomático de IL. El tratamiento sustitutivo está infrautilizado en nuestra muestra.

Summary

Introduction and aims: Lactose intolerance (LI) is known to have nonspecific clinical features that can be similar to other gastrointestinal disorders. It is important to know the clinical and epidemiological features of patients diagnosed with LI as well as its important to describe its predictors, the association with other pathologies and the use of a replacement therapy.

Material and approach: Retrospective study of patients with LI diagnosed with hydrogen breath test for lactose intolerance (positive result in the test was named: HBTL +) from 2010 to 2014. Clinical and epidemiological data were collected as well as the information related to the replacement therapy.

Results: The study comprised 2038 patients, of whom 30% had HBTL +. The average age of patients was 35. In the multivariate analysis, the results of HBTL + were a 33% higher in men (OD 1.33, 95% CI 1.03-1.71). There is an increase in the number of tests carried out until 2012 and a progressive decrease in the incidence of HBTL +. 42.8% of patients with HBTL + were initially diagnosed with irritable bowel syndrome (IBS). 33.8% of patients with inflammatory bowel disease (IBD) had a HBTL +. The main symptoms for which the test was indicated are abdominal pain and diarrhea. While performing the test, the symptom with higher cases leading to HBTL + was bloating (35% p = 0.03). Prescription of replacement therapy was done in 36.5% of patients with LI.

Conclusions: LI has a moderately frequent occurrence in our region of 30%. The average age of patients suffering from it is 35 and is more common in men. There is a progressive decrease in frequency despite the increase in hydrogen breath tests done. 42.8% of patients with HBTL + were initially diagnosed with IBS and a third of patients with IBD showed a HBTL +. Bloating is a symptomatic predictor of LI. Replacement therapy is underused in our sample.
